PAPERWORK DROPOFF DATES

The following items must be submitted along with the 2020 registration fees before any participant can start practice on Monday, August 3rd:

1. 2020 Physical Form (PDF).

2. 2020 Participant Contract and Parental consent form (PDF).

3. Original Birth Certificate with raised seal (for new players only, please also bring a copy).

4. Legible and clear COPY of participants Final report card with ALL marking periods. Please do not give us an incomplete report card! We collect reports cards for all participants in pre-K and above (that's over 150 students) calculate the GPA and file the copy in our Team Books for Certification by the League. We can not track down parents to return originals. Please do not include envelopes or statistic reports for reading, math and district tests. Also, if a student transferred mid-year we need report cards from all schools attended.

5. COVID-19 Waiver for Football (PDF) or Cheer (PDF).
